Around us, there are many people who appear cheerful on the outside but are silently battling depression. These individuals often struggle to identify their own feelings of sadness while dealing with depression, a phenomenon known as "smiling depression." This condition progresses through a serotonin imbalance in the brain, yet the affected individuals exhibit bright behavior externally, masking their inner turmoil.

As awareness and understanding of smiling depression continue to grow, more individuals can break free from internal struggles and seek the help they need.

Those with smiling depression undeniably experience feelings of sadness and are in need of treatment, yet they may not recognize their own depression. At times, they may be misunderstood as experiencing "hypomania" due to excessively cheerful behavior. This condition is particularly common in individuals who have difficulty expressing their usual feelings of sadness, possess strong self-esteem, are highly conscious of others' opinions, or tend to suppress their emotions. Moreover, it frequently manifests in professions where maintaining a positive demeanor is essential, such as emotional laborers, entertainers, and flight attendants.

Smiling depression can manifest not only in mental symptoms but also in various physical manifestations. Headaches, sleep disturbances, chest pain, increased heart rate, fatigue, loss of appetite, and digestive issues are some typical physical symptoms. Consequently, many patients end up visiting various medical specialties, occasionally leading to misdiagnosis as a "functional disorder." In elderly individuals, smiling depression may even manifest as "pseudodementia," with a potential for rapid decline in memory due to the depressive state, which could improve with depression treatment.

Therefore, if there is no clear medical explanation for physical symptoms or if symptoms persist despite relevant treatments, it is advisable to consult with a specialist in psychiatry and undergo a depression assessment. Treating smiling depression begins with acknowledging one's own depressive state. Cognitive-behavioral therapy, medication using antidepressants or anxiolytics, and counseling are often employed in the treatment process. Lifestyle improvements are also crucial, including engaging in external activities and exercise 2-3 times a week and ensuring an adequate amount of sleep.
